I'm with Troy in this. I prefer #1 over #2's lines but I liked the off-set name logo better than the balanced one. Of the off-set designs, #4's was easier to read when I pushed my chair back from my desk, but the rest of #3's layout reads better, with the phone number popping out more clearly.
I'm curious also about including both addresses on the card. I can see the need on invoices and letterhead, but on business cards I'd think you'd want to use that space to provide other memorable details like affiliation logos or details of services provided.
I have to be honest also and say that the letters "D", "C", "T", and "K" jumbled together at my first glance and I read them as "DICK". I'm pretty sure that it's not a Freudian thing so maybe it's an optical illusion/subconscious thing worth considering?

Not a fan of business cards that are printed in portrait instead of the more normal landscape orientation. For those folks that organize them into a Rolodex they are a pain in the butt.
#2 logo, #1 name below the logo is better.
Not a fan of the separate addresses, pick one for the card, preferably not the PO box.

Dan, some good points here, I posted Marge's quote because I feel it does catch your eye, depending on which one the eye doctor is asking you to cover, reminds me of an eye chart. Jus saying and I agree with Shaughnn about the letters.
I think you got too many letters and numbers going on there, nothing really stands out. I would have something on that card that jumps out to say hey I do tile work, without trying to read all the letters, numbers, too busy. Import something into your card where a person can see you do tile work without having to read anything. Member, Keep it Simple!
