Can someone identify this tile?

I've got about 12 tiles that need to be replaced. At some point, the last of this batch got used up, and we don't know where to find it (or one like it). What would be the industry description of this style? Anyone know where to find it in Phoenix? Any help is much appreciated.

BTW, it was originally from Home Depot. So if anyone knows of a liquidation store that might buy from them...

It's 12x12, this piece was removed from another room at some point, so there's nothing on the back that can be read anymore.

probalbly not fred sorry . tiles get discountinued very often. and we dont even know the name of it

I would think your best bet would be to take it with you and try and find a close match. Even if you had the name of it, from 5-7 years ago there would be a different dye lot even it was still made. Something from homedepot though it is unlikely it is even still made.

You'll have trouble matching tile 6 months later, let alone 6 years.

Go for a close as possible match, or take out enough tiles to make a pattern
