Floor Prep for Ceramic Tile

I wish to install ceramic tile in a kitchen that has a 3/4" thick tong and grove wood floor. The floor and joists are basically in good condition, except for two details.

1. When I removed a parquet floor covering, I discovered a strange situation. It seems that plumbers, rater than core through a joist, ran a pipe up, over and down the joist, thus creating a 3" by 3" void on the floor to clear the pipe. There is now the question of whether I may "bridge" this void with 1/4" cement board or do I have to install 1/2" or 3/4" plywood and 1/4" cement board over the 3/4" tong and grove floor as a base for a tile floor covering. I like to accomplish the job with as low a step as possible between rooms.

2. The floor is not level. In order to help level the floor I will need to "feather in" some additional thin-set to fill the low points in the floor (about 1/2" to 1" maximum thickness). May I add this extra material below the cement board or is it best on top of the cement board? I do not want a "mud job" because I wish to have as low a step as possible between rooms.

Your best bet is to post pictures. I will say that 1/4 in. backer board won't bridge much of anything. It needs solid backing beneath.

You will need to know the size, spacing and span of your joists so you can run them through our deflecto.
