You can certainly use a grinder with a diamond blade, but you may need to cut the back after the front cuts. Obviously, a hole saw is preferable if it is of the right size.

Up near or above the shower head since this sees typically little water spray. Not ideal, but for something that small, debatable if it really requires Kerdi removal and patching....

I generally do NOT grout the jambs. I always undercut them and think that looks more professional...but leave a very small gap between them and the tile.
