I have always used sanded grout for joints 1/8 and larger. Make sure you clear out enough of the old stuff to allow the new to bond properly.

I usually don't use backer rod for tile floor joints. I don't know how thick your tiles are or how much thinset is in the bottom of the joint, but I would just caulk it.
