They are a coin toss, personal choice.

We normally build our niches out of foam board and cut them in after tiling has started as to get the grout lines to fall perfectly in whole tiles.

The standard height for a grab bar is between 32 and 36 inches above finished floor height aka AFF. We normally use 2x8's to be assured the strike zone is hit.
