Do you level the subfloor and then apply the cement board? Seems logical.

Do you think if I force thinset under the flange I will raise it too much? And or get it out of level? Or if so, should I be able to adjust the tile a bit to compensate?

As for the plumbing below, I don't think there is an issue which would have caused it to...
